Thank you for choosing the Wild Health Fundamental of
Precisions Medicine Course (“Fundamentals Course”). Your use of the
Fundamentals Course is governed by the full Precision Medicine Training Program Terms. The following is
a brief summary of the full Fundamentals Course and Training Program terms.

Description:

Fundamentals of Precision Medicine Course: This is a six month self-guided course related to
precision medicine. Wild Health reserves all rights related to scheduling and modifying all courses and
content. You must be at least 18 years of age to enroll.

CME Credits. Upon completion of the Fundamentals Course, Wild Health will
provide a certificate of completion and you may claim 50 AMA PRA Category 1 Credit(s) according to the
terms provided by our accrediting agency. For more information about our certification and CME credits,
see our Training Program Terms.

Fees. Fees will be charged according to the schedule shown during the checkout process. Fees
may consist of an up-front access charge and/or a monthly charge. You authorize us to store your
payment method(s). If Wild Health has agreed to receive payments from you for the Training Program on
a monthly basis, you authorize us to automatically charge your payment method(s) the monthly charge
plus applicable taxes every month until the term of the Training Program concludes.

Cancellation. Cancellation requests must be received within 30 days to receive a complete refund of your
fee. For any cancellation requests received after 30 days, Wild Health will consider reasonable requests
for a refund of unused services in accordance with our Training Program Terms. Refunds will be provided
to the original method of payment. Cancellation requests must be submitted by the form available here.

Fundamentals of Precision Medicine Course

The Fundamentals of Precision Medicine Course is a comprehensive, self-guided curriculum designed to teach current or aspiring health professionals the basics of genetics-based Precision Medicine.

Fully asynchronous, this year long course offers participants 12 in-depth learning modules to prepare for real-patient application, along with the opportunity to engage with the Wild Health Education faculty. Participants are eligible to claim up to 50 AMA PRA Category 1 Credits™ for completing this course.
